Care to Learn Volunteer Policy & Waiver

Please carefully read Care to Learn's Volunteer Policy and waiver below. This is required for all volunteer participation.

I wish to volunteer with Care to Learn at the Care to Learn Central Office site located at 1047 S. Glenstone Ave, Springfield, MO 65804.

By submitting this Volunteer Waiver Form, I acknowledge that I waive any and all medical, civil, or other claims against Care to Learn arising out of the performance of my volunteer duties, whether for an on-site work-related injury, personal injury, or otherwise.

I assume all liability in the event that I am injured while engaged in volunteer work for Care to Learn. I attest that I am covered by a valid medical insurance policy.

I understand that I am expected to engage in volunteer work with Care to Learn in a reasonable and safe manner, and if at any point I cannot engage in volunteer work in a reasonable and safe manner, I am to alert Care to Learn Central Office staff immediately. Additionally, I agree to disclose any accommodations required.
